Radiotherapy | - Fax consult to secretary (5919)
- Phone secretary to confirm (5901)

Neurology | - Only request a neurology consult only if specifically requested by admitting consultant
- Use e-referral system
- Detailed consult expected, including collateral (e.g. witnessed collapse).
- Bleep the Reg if urgent

Geriatrics | General Consults Consults are sent depending on the patient's address corresponding to the rehab units: - Lyons - Camillus - Limerick City
- O’Connor - St Itas - West Limerick
- Peters - Thurles - Tipperary
- Locate a PC with the EPMS (Filemaker) referral system installed on it. Fill out consult to the appropriate consultant, then bleep their registrar.
- EPMS found on PCs on 1D, 3D, 3A, 1B, 3B, 3C, 4C (red sticker on PC)
- Social history is important (baseline mobility, independence, and cognitive collateral - see IQCODE)
Rehab St Camillis: (Limerick City and environs) - E referral and call Prof Lyons reg.
Rehab St Itas, New Castle West: (Anywhere West Limerick, Killmallock-Patrickswell/Kildimo/Abbeyfeale) - E referral and contact Dr O’Connors Reg.

- For suspected cases of glomerular disease or where kidney biopsy might be warranted, contact Dr Casserly or a member of his team.
- Specific forms available e.g. AKI from on ward
- Check notes to see if known Renal patient of Prof Stack and Dr Casserly (look through clinic letters)
- Urine Dipstick for blood/protein before contacting Renal Reg. Specialist

Palliative Care Team | - National Palliative Care Referral form used available on wards and at www.hse.ie/palliativecareprogramme under resources
- Bleep the palliative care nurse with consult request

Pain Team | - Use e-referral system
- Very approachable team, if you have any queries just bleep the pain team CNS on 330

Microbiology | - Contact Reg on bleep 626 or the secretary at 5099
- Consultant to consultant out of hours
- Detailed history vital; admission, bloods, antibiotics used and response, vitals, culture/swab results (esp previous results), type of surgery, prosthesis or pacemakers, indwelling catheters/drains, MRSA/KPC/ CRE contact
- Have their vitals to hand and know whether they are clinically well or not

Cardio | - Use e-referral system
- Completed before 11 am will be seen that day
- After 11 am will be seen the next day
- Fill out forms with the relevant information

Psychiatry | - Fax consult to secretary - 5114
- Call the secretary on 5092 to confirm
- Old age Psych fax for referrals – 061 48 3781

Dermatology | - Bleep Dermatology Reg. discuss the case.
- Drop consult into Dermatology department, 5th floor new building, near canteen.
- Dermatology has requested all consults to go via consultant to consultant. They sometimes accept from a Reg. If it is urgent don’t be afraid to contact the Reg.
